Client Health & Preferences Form

If you are a first time client or if you have any health changes since your previous visit, please complete or update your Client Health & Preferences form prior to your visit and review the Cancellation, Reschedule, No Show and Unexpected Illness Policies listed below. If you are unable to complete the form ahead of time, to allow enough time for review of your information prior to your appointment. 

Shared Commitments, Terms & Conditions During COVID-19 Pandemic

Being indoors and within 6 feet of others increases risk of exposure. While properly fitted face masks, increased ventilation/airflow, cleaning/disinfecting protocols and other precautions may help to create a safer environment, they do not remove risk entirely. Please review the Shared Commitments, Terms & Conditions During COVID-19 Pandemic.  Clients must understand and assume risks associated during time spent indoors in the suite and massage studio prior to booking their appointment.  

About your Therapeutic Massage Appointment

Your customized therapeutic massage includes your full scheduled time for your massage as well as additional time for consultation before and after. Your massage may also include heat/thermal therapy, aromatherapy and assisted stretching based on your preferences with no additional fee.  Our office has a comfortable waiting area with complimentary hot teas and filtered water. Feel free to arrive up to 15 minutes early to wind down and relax before your appointment.  Doors to the suite may be locked should you arrive more than 15 minutes prior.

Updated Unexpected Illness Policy

As always, please do not visit the studio if you have any cold, flu, flu-like symptoms or other communicable illnesses.

The Unexpected Illness Policy has been broadened until further notice.  If you have any cold, flu, flu-like symptoms or other communicable illnesses or if you have been in contact with anyone with these symptoms, please reschedule your appointment for 14 days after your scheduled appointment date or after symptoms have cleared, whichever is longer.  Additionally, any immuno-comprised or medically fragile or clients who prefer to practice social distancing may reschedule or cancel without a rescheduling or cancellation fee. Before your rescheduled appointment a complete intake, consultation and screening will be completed to determine whether massage is appropriate.

Cancellation/Reschedule Policy

I understand that unanticipated events happen occasionally in everyone’s life. In a desire to be effective and fair to all clients, 48 hour advance notice is required when cancelling or rescheduling an appointment. This allows the opportunity for someone else to schedule an appointment. If you are unable to give 48 hours advance notice, your fee will reflect the full scheduled session. Meg Donnelly LMT, LLC reserves the right to refuse future bookings as a result of multiple cancellations or reschedules. 

Unexpected Illness Policy

Please note that for the health of myself and other clients, clients with any illnesses are required to cancel their appointments if contagious.  Additionally, clients with cold, flu, flu-like or other similar upper respiratory illnesses should reschedule any appointments that fall within 14 days from the first sign of symptoms or after symptoms have cleared, whichever is longer.  Other illnesses that are not contagious in nature may or may not require cancellation or rescheduling.  When in doubt, please contact me ahead of time as soon as possible, so we can discuss.  Late reschedule fees will be waived if client moves appointment to a new date/time due to illness. Additionally, if client is unable to reschedule due to the circumstances surrounding their illness, please contact me directly and a one time waiver of cancellation fee will be applied. 

No Shows

Should you forget or consciously choose to forgo your appointment for whatever reason, it will be considered a no-show and your fee will reflect the full scheduled session. Meg Donnelly LMT, LLC reserves the right to refuse future bookings as a result of multiple no-show appointments. 

Late Arrivals

If you arrive late, your session may be shortened in order to accommodate others whose appointments follow yours. How late you arrive will then determine if there is enough time remaining to start a treatment. Regardless of the length of the treatment actually given, your fee for services will reflect the full session.
